ILR and PEO appointement

I need PEO appointment from 23/oct/12 to 23/Nov/12 time frame for ILR. My entry clearance was 24 Nov 07 and I am on tier 1(hsmp) general visa.

If I try book online I get no slot available, I read other threads mentioning 42 days limit, Not sure if this is the case? I see that only dates available is 3 months advance. Any one getting success for that time frame. Anyway I don't get any slots any where even I try to book.

I will start to do this midnight now on, What information will required if I get a slot? I see some CASE ID(help section from ukba site) might be required? I need this information in case I get a slot but don't have full info. Thanks in advance for your comments.
