Tier 2 Workier and wife switching as depandent on me.

Dear all
Friend of mine has the situation as follows.
He enter the country as depandent of tier 4 student. Stayed here for 26 months and switched to tier 2 general visa. Now he wants his wife, a tier 4 student, to be depandent on him for visa purpose but does she need to leave the country and apply for fresh application or it can be done within the UK.

Any advice will be greatly appreciated.
