IMPT:Question regarding Immigration History (form section D)

Hi follow members,

I need help filling up Section D Immigration History of Tier 1 General form

History:
I came into UK on Tier 1 Entry Clearance visa (issued in Oct-10) on a previous passport. The passport was stolen couple of months back in France and along with that visa was also gone. I got new passport (Indian) and new entry clearance visa from British Consulate in France on this new passport in Feb-12 and then came back to UK.

Questions:
Now please advise me that for question D3 and D4 should I give information from new entry clearance visa or original entry clearance visa (which was stolen)?

D1. Did the applicant obtain entry clearance or a visa before entering the United Kingdom?
Answer: YES

D2. Where did the applicant obtain entry clearance or a visa before travelling to the United Kingdom?
Answer: INDIA, MUMBAI

D3. What date was the entry clearance or visa issued?
Answer: ?

D4. Please enter the reference number on the entry clearance or visa which can be found in the applicant’s passport or travel document.
Answer: ?

Answer to D3 and D4 will also provide answer for questions D5 to D22

dont you have a photocopy of that old passport ?

also if you don't have then you might need to attach the paper work you submit in france.

I have photocopy of previous passport and visa and that's not the concern. I am just confused that which one I should use for filling up this section?

I would say mention the old (if you know it) and the new one both in the application form and include one covering letter where you can mention all this.
