Please help me in making ILR application

i have been living in the uk since last 10 years and going to make set(o) application after few days on the basis of long residency.

what documents should i attach with the application form?

one thing more i lost my first two passports in november 2009 which i reported to police and make the new one; and later stamped my visa on it. now i have two visa on the same passport.

i read in the application form that i need to submit my all previous passports which i dont have so what should i do?

Submit the passport currently you have and I would include a cover letter explaining the lost passports and attached any letters from UKBA regarding lost passports

i did not get u where i have to look for section 11......sorry i am using first time this board so i dont know how to find things.

further if u can help me the list of the sujjested documents to support my application.
