long residence application help please

Hi, I am making my long residence application soon (10 years category) and hope someone could help me with the following questions:

1). on the Set(O) form section 10, question 8: give details when your fingerprints were taken. I really cannot remember the date. My Biometric card have got the issue date: 03/11/2010, so will it be ok if I put down "October 2010" for this question?
2). I saw some people would include a cover letter in the application, is this necessary?

2). I saw some people would include a cover letter in the application, is this necessary? Why?
