Tier 1 (G) extension previous earning Question

Hi Guys,
I need enquiry from my previous earning. I have tier 1 G extension next month.
I am showing my earning payslips of last 12 months (sep 11- sep 12) from 1 employment. I am showing my current salary is £40k however 1st 4 months out of 12 my salary was £35k (from august 11 - dec 11) and £40k from (jan 12 to date). I put £40k as my salary in point base calculator and in visa extension form.

My question is do I need declare (change of salary) this in form or just simply say £40k as it is my current salary?

I provide 2 documents 1 for employment letter says my current salary is £40k and 2nd is letter showing 'Change of Salary'.

Please let me know if i need to declare my both salary in form or just mentioned my most recent salary...

The previous earnings have got nothing to do with what you current yearly salary is.
You have to add up the gross income that you have in the 12 payslips (in your case Sep 11 - Sep 12) and that is what you have to use in the points calulator and mention in the form. You dont need any "change in salary" letter. UKBA are only interested in "previous earnings" not your package.

you need to calculate total salary that you have earned to ensure you are getting enough point...UKBA doesn't consider how much your current salary is but it cares about what you have earned in past 12 months...
