Family Visit Visa Question - Urgent

Hello Everyone,

I am applying family visit visa for my sister and her daughter. I am trying to fill her online application form. I am bit confused, do I need to fill two separate application form (VAF1B) or just one application form is enough. Her daughter is only 5 months old. Please Advise.
