Ltd Company - pay slips questions

Hi,
Does anyone who applied for tier 1 with their own ltd company have a template of pay slip which is accepted by UKBA.

is there an online portal or software which any one have used in the past. The one my accounttant provides is just a word document should the gross/net pay and NI etc.

My questions are :

-does it need to be in my company's letter head ?
-Does it need to be signed and attested by director of company ( thats me ofcourse) or the accountant ?
-does payslip need to be stamped with the seal of my company which shows reg No and VAT no etc.
--does dividend voucher need to be stamped with the seal of my company which shows reg No and VAT no etc or does accountant has to certify that...

No stamps needed

I was approved with payslips from my own company today and they were just PDFs from my accountant - no company letterhead or stamps involved. They did look like payslips though (ie looked like payslips I have received from other employers) so I'm not sure if your Word doc will do. I think looking for a template is the right idea, but once you have that, my experience today suggests you'll be ok.

I had signed my dividend vouchers, but not stamped those pages. I had a letter from my accountants certifying it, as well as bank statements showing the transfers - they asked to see the vouchers and the bank statements but did not need the accountant's letter.

I'll post something on my whole process on this forum soon, so look out for that in case that's helpful with other questions.
