ILR Set (O) work permit + HSMP Tier 1 General

Hi ,

Can any one help me about the process to get ILR through set (o) form?
My 5 yrs finishes at December 5. I was on work permit around 1 and half yrs and remaining period I passed as HSMP Tier 1 (General) point based system. I need help regarding Premium service. I want to know following things,
1. What documents are required?
2. Any help in Covering letter?
3. What documents are required for my spouce/wife?

Any help would be appriciated.
