Tier 1 Dependant visa from India

Hello All,

I need to file for my wife's dependant visa who is currently in India. Could some one please answer my query?

1] There is an Online application form, there does not seem to be any option to select Tier 1 dependant, the only option is "Settlement". Could someone please let me know what kind of option should I be selecting? The document mentioned for "settlement" VAF4 and for Tier 1 dependant it is "VAF10". Can someone please confirm what I need to select.
2] My wife's name in passport has not yet changed. Can I put her earlier name and later after her passport name change is completed I put her changed name? Can this change be done later ? when we go for the appointment? or this cant be done later on.
