query regarding dependent appt booking

hi folks

i am in a bit of a problem. when i booked online last time for my set (o), i booked online for myself only and i did not book for my dependent(wife).

what are the options now?

1. do i have to rebook another appointment with dependent
2. can i bring dependent also on that day despite not registering her on the appointment
3. do i have to rebooked a seperate appoitnment for my wife only
4. any other suggestions please
