Doubts in the new PBS Dependent form (TIER1 Extension)

I have few doubts in the latest PBS Dependent form –

1. M6 (pg 31) – Has the main applicant’s application been assessed in accordance with paragraphs 39-42 of the HSMP Forum Ltd. Judicial Review Policy Document ?

• a. Yes / No ?? What should I tick here please ?

2. L3 (pg 29) – Reference number of the main applicant’s most recent application to us :

• a. I do not have any ? Can I leave it blank please ?

3. L4 (pg 29) – Date of the main applicant’s most recent application to us :

• a. Very confusing as we all are applying together for extension ?

4. B9 (pg 10) – If you are known, or have been known, by any other name(s) than those given above, please give details & send evidence:

• a. Here they are asking for Dates from and to ? But, all the names are valid till present. So, should I put something like –
  • i. Name A – Birth date till present ?
    ii. Name B –Marriage date till present ?
  b. And, regards to evidence, I have marriage cert & an affidavit from India. Hope that would suffice ?

Can somebody help me please ?
