Access Right To A Child

Hi Guys

Am applying for access rights to a child from within the uk
One of the requirements is a sworn affidivit from the child's mum to say i have an active role in the child's life and one from me to say i intend to continue active role in the child's life.

Can any1 help with how and where i can get a sworn affidivit done? and does it has to be notorized?

Am doing the application myself without an immigration solicitor, is it only an immigration solicitor that can help with the sworn affidivit?

I've looked into this abit further and can I just use a stand general afividat form online for the above? But will I need to notarise it? Plz some one reapond. Thanks

Family life as a parent of a child in the UK wrote:a sworn affidavit from the UK resident parent or carer of the child, the affidavit must:
o confirm the parent applying can have access to the child
o describe in detail the arrangements to allow for this, If contact is supervised, the supervisor must swear the statement, and
o be sworn before, and certified by a legal officer.
Consult a solicitor or a notary public.
