Tier 1 Form Queries

Can some one please help for following two queries on Tier 1 Entreprenuer Form:

1- On the application form page 28 what does this line mean ? “The applicant should tick one box only to indicate which group he/she is in:” Where are the boxes that they want us to Tick???? Please let me know page number if there is any? I can see only Bullet points.

2- The third party has nothing to do with Section 5 of the application for Tier 1 entrepreneur ? where they ask “Name and address of joint customer”

It could be because of a misprint, have a careful look at the form again. I have not personally seen the form so can't be so sure.

A new form is released today, it has now got the relevant boxes.

Thanks. Yes the new form has the boxes now.
Please respond to my second question also, whihc is: The third party that is helping in £50K has nothing to do like "sign" in Section 5 of the application form?
