ILR Eligibility ? Please help

Hi, I am a longtime lurker on this forum and have gathered a lot of knowledge from this forum, so really appreciate everything given by the members. I am now close to completing 5 years in UK and was wondering whether I would be eligible to apply for ILR. Really grateful if the knowledgeable members could help me out with my 2 questions.
1. Am I eligible to apply for ILR ?
2. How will they verify my out-of-UK dates without old passport.

Here is my a brief timeline for those 2 questions
  • 1. I came to UK on work permit on 27-Dec-2007.
    2. Went to Holiday in India - 25-Jul-2008 for 16 days (GAP)
    3. Renewed workpermit on 25-Nov-2008 in UK
    4. Went back to India on 9-Feb-2009 and due to change in employer had to apply for VISA again and got Tier-2 ICT from India - (GAP of 53 days - UNPAID)
    5. Lost my passport on 30-Mar-2010 and got a new blank passport.
    6. Applied to Tier-1 General on 4-Apr-2010
    7. Went back to India on 13-Nov-2010 for 19 days
    8. Went to India on 25-Dec-2011 for 15 days.
    9. Had other holidays outside UK for 11 days
As per the 28 day rule, am I eligible to apply for ILR on 28-Nov-2012 ? i.e. Will point number 4 be considered a break in my settlement ?
Since I lost my passport, how will they verify all the old dates when I went out of UK. ? I have only been out of UK for a total of 114 days in 5 years.

I can satisfy other requirements like PBS, funds, cohabitation etc.
