Tier 2 ICT Dependent Visa Documents

I need to get Tier 2 ICT Short Term Dependent visa for my wife. What documents are required to apply for visa? If her passport contains my name (under Husband's Name), will that be enough or a marriage certificate is a must? Can a court affidavit regarding the marital status be considered enough?

unfortunately i cannot find the list of all required documents

could you please list down the mandatory docs (especially about marriage)
