first time applying inside uk Tier 2 please help

Hi

I am really stuck in few question it will be very kind if you help me

1. Section H mentioned about reference no . What is that? I never receied any letter from UKBA or Home office as per as I know. But I have granted three times visa before in different categories

Can you please help what should I write there

2. Section J asked about previous addresses but there is only one space there , I have three previous address how can I write them?

3. In sponsorship licence number ( M1) there are 8 boxes but my sponsors licence is of nine letter

Please reply asap . I am going to submit it in few days
