Requirements of ILR based on spouse visa !!

Could anyone tell what are the requirements of ILR based on spouse visa.
What documents are mandatory to attach to the application.

Secondly how to book an appointment for same day service, I could not book it online whereas their enquiry no. is not responding.

Any suggestions/tips would be appreciated.
