Extending discretionary leave to remain

My friend who was given 3 years discretionary leave in 2009 is due to renew it and does not know what form to use. We are not sure if it is form FLR(O) or HPDL. Does anyone know which is the correct form to submit the new application for extension?

She had originally applied as the spouse of someone settled in the UK but her application was refused because she had overstayed her visitor's visa. Also her husband had died a few months before HO requested a few documents from her which she could not supplied. She wrote to the HO informing them and her case was refused with the right to appeal. She was later granted 3 years discretionary leave after appealing in court.

We would really appreciate if someone could point us in the right direction as to which form to use to extend the discretionary leave?
