Documents required for 10 Years Long Residency in the UK?

I am going to apply my ILR on the basis 10 Years Long term Residency in December 2012

I would like to know what documents I need to submit with my application?

Still bit confused what I will be needing for 10 years long residency. I doubt that I have to submit my 5 years employment etc.

has someone previously applied for 10 years London residency visa?
