Tier 1 Extension Address Issues

Hi all.

I am due to apply for my extension, slight problem that I moved address and didn't get it changed on my bank statements to the new location.

However, in the form, there are two places where it could be an issue:

1. Payment details, the card might fail as the correspondence address is different from the billing address.

2. Section 5 - authorization to contact the bank. Again, different address required here.

Has anyone had this problem? What did you guys/gals do?

Also, do I need to photocopy the documents and attach?
