On a recent booking, due to technical glitch I successfully but not intentionally booked two appointments but neither of them showed on my UKBA account. I emailed UKBA and below is their response.
FYI, my appointment still does not show on My Account at the UKBA website.

On checking the appointment calendar we can confirm that both appointments were booked. Therefore, we have cancelled the appointment at Xxxxx PEO on 5th December 2012 11:30 as requested.

On 24/10/2012 Wednesday early morning (between 0010 and 0030 hrs), I tried to book online appointment for SET-O Premium with one dependent. Once slots were available, I picked one and entered my details. However, At the final stage the screen 'blanked out', and I got an error message. I tried all over again, and got to the same situation.
On checking my email, I found these two confirmation of appointments:
Booking reference: 16-2-102rrrrr
Date: Vvvvv Nov 2012
Time: Yuyyy
Location: Xxxxxx
Number of attendees (including yourself): 2
Booking reference: 16-2-102xxxx
Date: Wed 5 Dec 2012
Time: 11:30 AM
Location: Cardiff
Number of attendees (including yourself): 2
Could you please confirm that either of these are actually booked?
If only one is actually booked, could you tell me which is the successful one?
In case both are booked, I would like to retain the bbb Nov 2012 one, and cancel 05 Dec 2012.
