Question regarding the tier-2 6 years limit

Hello,

I'm wondering about the following:
Is it clear when the count for the 6-year tier-2 limit starts? In other words, imagine someone who got a T2 ICT "entry clearance" in April 2011, then went back home and applied for a T2 General "entry clearance" in April 2012.
Would the 6-year count start from April 2011 or April 2012 given that the second visa was an entry clearance as well not a leave to remain.
It's worrying if they start the count on April 2011, because it means any possible T2 general extension would be limited to April 2016 instead of 2017.
I'm asking this because if time comes for ILR, they would start counting 5 years from April 2012 not April 2011. So this makes the 5-year count pretty tight if the T2 extension ends on April 2017.

Any advice please?

does the 35k rule apply to all 5 years....lets say I was earning 30K in the first four years and in the last year I earned 35K ..am I eligible for ILR
