I encountered this issue so wanted to share this with evryone. My wife already had her UK residency (dependant of Tier 1) from April 2012 (so pre July 2012 rules apply). I changed my status to ILR in Aug 12. For my wife I filled up the FLR M form which was accepted by the Croydon PEO. However, one of my close friends (same case as mine) was asked to fill FLR O form at Solihull PEO.

I wud advise that anyone applying thru PEO same day, fill in both the forms. No harm in doing that but saves the hassle.

A case of ignorant caseworker, nothing more.
FLR(M) is the appropriate form if a PBS dependant wishes to extend leave under family route when the principal migrant applies for / has received settlement.
