Tier 2 UKBA premium documents checklist

Hi all
I'm switching employers, my first Tier 2 General visa was granted to me in India at UK consulate. I'm changing employer and I booked my premium appointment at Cardiff next week, could you guys please let me know checklist of the documents.
And any helpful hints you could give me, thanks a lot! I'm about to fill online application for Tier 2.

I didnt had a national insurance number for the last 6 months, I am switching employers and filling Tier 2 application.
Is national insurance number required to fill in the application? will it have any complication if I dont apply for one before I go for my intw with UKBA formy new Tier2?

If you ever have, or are, working in the UK you must have a NI number. It is shown on your payslips.
