Tier 2 _change of employer

I am currently on Teir 2 and have been offered a new position by a different employer, which I plan to accept. My question is which application form do I use? Will it be a completely new application or an extention? I will appreciate any help on this. Many Thanks.

It's a new application but you would use the same application form in either case. You simply follow the instructions related to your circumstances. Make sure you read the information on the UKBA site before you apply.
