PES Online booking - what personal information is needed

Hi,

I am trying to book the PES and am going to engage a couple of family members to help. I presume that it is all right if they all create their own accounts (username, password, secret word).

I know all the steps - but have never reached the enter personal detail screen as I was unsuccessful always.

What personal information do I need to enter (e.g., name DOB passport number) just so I can ensure my family members and friends have it if they reach that screen.

As I recall it, all that is required is the applicant's first name, last name & date of birth. The applicant's contact number is pre-populated on screen (from the account details). The passport number is not required.
