EEA3 application advice please

Hello.

I need of some advice, please.

I've arrived to UK on 6/03/2008 with my wife and I've started work in April 2008. My question is when can we apply for EEA3 Family Permanent Residence? Is it a date when we came to the UK or when we started work? We are Polish citizens of A8 countries.

I've started work on 6/04/2008 as self-employed until October 2009 and after that I've started work as employee for the local company. Unfortunately after 5 months I've applied for WRS Certificate (received March 2010 ), how they can treat this gap, as a period of unemployed or it doesn't matter? On this company I've worked over 12 months without any gaps with WRS. Currently I am working for different employer.

In the next year I would like to apply for EEA3, I'll be the main applicant, included my wife in Section 2 as a family member. Which sections do I have to fill in and does my wife have to attached all documents of treaty rights ( e.a. P60, letter from employer, bills etc. )?
