Tier 1 Dependant visa

Hi,

I am about to apply for my Tier 1 extension and dependent visa for my family. I have one question regarding question L4 of the dependent visa form.

L4. Date of the main applicant's most recent application to us.

The thing is that I don't know the exact date when I sent my last application. I just know that it was around last week of November 2009. Can I enter an approximate date in this question or do I have to enter the exact date?

Could anybody help me on this matter?

I also noticed that the form for Tier 1 extension has changed? Could anybody tell me if there is any major changes from the old version?
