Hi
My child born in UK in the month of April 2012. and my visa valid till August 2013. I want to apply my child visa so which form I need to fill and how much fees and what documents required for it.

You will need Tier 4 Dependent form and all those documents which you use for your spouse plus child's birth certificate.

You will need to make sure that you have maintained required level of funds.
