Is the basis of your 12 month period starting with the first payslip or the first credit into your bank? You could only be in situation to declare 13 salaries if they we all actually credited to the bank within a fixed 12 month period. It sounds like this last one is falling outside that period, which may be okay, but only if you have not included the first payslip in lieu of how you are counting the date of your 12 salaries (payslip date or BGC date). You can't have your cake and eat it!

Is this last payment vital to your points? If there is no other option, I would say use and see what happens.

All salaries from Dec 2011 to Oct 2012 were credited on bank every month 27 or 28th, Only Nov 2012 will be credited in the December 15th.

I can't see t this being a problem is you are claiming only 12 salaries throughout the 12 month period and the actual payslip was issued within that period.
