Third Party Funding From Pakistan (Solicitor in UK Letter)

Hi,

My friend is showing third party funding from Pakistan. And the affidavit says it needs to be signed in front of SRA regulated solicitor in the UK. Do you guys have any idea for a sample template that the solicitor needs to write on their headed paper and sign/stamp?
