URGENT Need Help... Tier 2 General

Hi I am working in a company for the past 3 years Last year i have switched from PSW to Tier 2 general using UCOS and they have provided me only 1 year due to mistake and now my visa is going to expire in February and I have asked to generate another UCOS and have applied for my extension again for another 3 years under the same employer and same job and same SOC code with same salary. I have used the Tier 2 General Application Form Again Online And Paid The Fees And Sent The Required Documents. Now I had a fear whether i had used the correct application form. Do I need to use the Tier 2 (W) extension form.

I need help soon. If i am wrong what are my options as my visa ran out Feb 23 2013
