Tier 1 General visa extension with dependent old rules

Hi Folks

Bit confused and would really appreciate your help re below

I was granted Tier 1 General visa as per rules that were in place prior to 30 March 2009. However the visa was only granted in Feb 2010 for 3 years.
A year later I was joined by misses.
Now I am going to apply for the extension and got a peo appointment in Feb 2013 in Solihull. When booking the appointment I selected Tier 1 General and included my wife as dependent.

My question is
1) Should I had booked a separate appointment for my wife or would my existing appointment that includes her as a dependent would be sufficient?
2) I have the extension form version 12/12 which says only for the use of main applicant. Does this mean I need to fill another form for my wife? Does anyone know which form would that be?

Any help would be greatly appreciated

Hi,

1. The appointment that includes her as a dependent will suffice. You do not need to book another separate appointment for her as she will attend the appointment with you.

2. A different form must be filled for your in country dependent and it can be found here
