Invoices query for ltd company /12/12 version dependent form

Hi there
I am applying my t1 general ext next week. I am taking income through two sources.
1- as director of ltd company
2-through employment (I am working as employee in some other company not my own ltd company)
My query is - do I need to send invoices with dividend vouchers to ukba or just the dividend will be enough.

Also pls mentioned where can I find 12/12 version of dependent ext form for tier 1(my wife and daughter already in the uk).i find 12/12 version of main applicant form but can't find dependent form (policy guide for dependent 12/12 version is available at ukba web site)

It seems the dependent form did not change and you should use the previous version.
