OCI Documents and photocopies

I would be grateful if someone could confirm if following documents are enough for OCI:
1. App form + photograph
2. Original + photocopy of Surrender certificate
3. British passport firstpage photocopy
4. Naturalization certificate photocopy
5. Original British passport/cancelled indian passport/naturalization cert (for checking?)

Have i missed anything? Do i need anyother photocopies?

In the HCI London website, it says

Photocopies of the first page, photo page, all observation pages of valid British/Foreign Passport

The first page of my british passport seems to be some standard text. The only relevant page is the photopage and on the same page on other other side text with no observations. Isn't this page alone enough for photocopy?

The only relevant page is the photopage of your british passport.

There is no need to submit any originals and only photocopies should be sufficient.

However you may carry originals with you just in case they want to verify.

Is the address proof required for OCI application?

Embassy and consulates in the UK have jurisdictions for applications, hence it is required to submit self attested photocopy of the recent address proof, if by post only. and same copy along with original to present when in person.

i recently applied in HCI,london in person and did not submit any address proof.
