AN form queries

I am applying for Naturalisation after having ILR for 12 months, and had some queries regarding the AN form:

1. Section 1.1 (reference numbers) - what is this referring to? When I received my extension of my Tier 1 visa, and subsequently my ILR the documentation refers to two different case numbers/id, therefore I am reluctant to write either here.

2. Section 1.17/Address history - for one of my residences, I do not have the exact date (only the month and year) that I moved in there. Is this likely to be an issue?

3. Section 3.2 - I am employed via a Limited Company of which I am also the director (this is not the same as being self employed for tax purposes). Should I therefore tick both employee and director in this section, and submit my last Self Assessment statement of account?



4. Section 3.5 - Address of tax office or tax office reference - what is this number? I am employed through a limited company, from which I have an employee PAYE reference on my last P60 - is this what I put here?

Any suggestions would be greatly appreciated, especially with reference to my employment, as I am unsure if the National Checking Service staff wil fully understand how Ltd Companies etc work.
