Tier 1 (General) Dependent Visa [ Bit confused, Need help ]

Hello All,

I have got Tier 1 General Visa, I am main applicant and I have got one Dependent ( my wife ). I have got some question and need your help.

Background :

Before our marriage, we were engaged, while my wife was coming from back home, at the air port, UKBA's IO officer stopped her and asked question about regarding cambridge college and hold her passport, and issue appeal and release her,

we got married here and we launch appeal , and after long time , finally she got visa on me, I mean she is dependent on me now.

UKBA hold her passport when she was coming from back home, so when she got visa, Home office called us and state that they dont have her passport and they could not find her passport and they will issue visa on STD paper. they did not send any letter regarding lost passport, they only did send her visa on STD. we make new passport and transfer STD visa on the New passport.

Sorry for long story, Now Question.

I am going to apply my extension and on dependent form, there is a question regarding previous passport ? I have to ticket on Loss and provide details regarding how did previous passport lost? so please help me that should I write , lost by home office and please provide reference no ,

I am a bit confused if they check record via reference no so they can easily see about appeal and regarding cambridge college. Will there any problem if they check about previous record of cambridge college? Can they refuse my wife's visa? What should I do?
