Help in filling up form T1 Extension

Hi,
I need a help in filling up the form as I am not sure about few sections.
Section 3 Page 27

-J9. Please provide full contact details that will allow each of the documents provided
to be verified if necessary. Please also provide here any additional information or
explanation of the documentation sent, that will assist us in our consideration of
the earnings being claimed. If more space is required, please provide the required
information in a covering letter.

Who detail I suppose to put here , is it ok if I put Operation Director details. Name mobile # etc.

SECTION 7 - SUMMARY SHEET Page 36
The section in table where is say "How Many?" for bank statement and pay slips should I put 12 Moths or ??

In second table where you show the point you claimed for each section in 3rd columns "Document Provided what should I put for UK experience and English Requirement.

1) The contact details you include depend on what evidence you are supplying for previous earnings and the person/entity that would respond to enquiries about them, if the UKBA made an enquiry. E.g. If bank statements, then include your bank branch address and contact details, if payslips then include your HR or Payroll department contact details etc...

2) English Language : If you are applying for the first time then you would need to provide a certificate for your English Language qualification, if you are extending your leave then you need not submit evidence again, so it would be left blank.
UK Experience : This would be proved by providing UK bank statements where you clearly show you have the required amount of UK earnings credited to it. (Usually the same statements you used for previous earnings can be used for this.)
