VAF9 Appendix 5: Tier 2 - Part 10 - Documents Provided help?

Hi there

I am currently filling in the VAF9 Appendix 5: Tier 2 application found on the following link

Regarding part 10 Summary Sheet

It request for Documents Provided. My employer has provided me a Restricted CoS based on the RLM Test.

Since the Restricted CoS is a unique reference number, what Document do I have to provide for this???

Also for Appropriate Salary, what Document do I provide for this? Do I need my employer to provide an employment contract?
