set (o) application fill out query

Hi seniors/experts

can you please help me with the following set o application form queries, i checked previous queries in the forum but dint find them.

1. i am going to pay with credit card at PEO , do i still need to fill out the payment section page 4 of set o form ?

2. do i have to attach all the 55 pages or can i take off irrelevant pages from it ?

3. do i need to carry the application form photo copy as well ?

3. do i need to carry the application form photo copy as well ? You don't have to carry, but highly recommended to keep a copy (photocopy/PDF) once all completed for your own record purpose.
