ILR dependant - Cohabitation

we are planning to apply ILR dependant visa 1st week of Feb 2013.

I have queries related to 6 cohabitation documents;

My wife joined with me 9th Dec 2010, since then we have following documents:
NHS Card register : 31/12/10 - Present (Both)

Teanancy Agreement:
-- 15 Oct 2011 - 14 Oct 2012, 15 Oct 2012 - Present (Joint)
-- 15 Oct 2010-14 Oct 2011 (My self)

Council Tax : May 2011 - March 2013 (Joint)
15 Oct 2010 - May 2011 (Myself)

Water Bill: 15 Oct 2010 - 3 Sep 2012 (Myself)
22 Feb 2011 - 3 Sep 2012 (Joint)

Electricity Bill : 12 Nov 10--18 April 2011 (Joint, but my wife surname not matching)
19 April 2011 - 14 Dec 2012(Joint)

Bank Statements : Dec 2010 - Jan 2013 (Myself)
18 may 2011 - 17 dec 2012 (wife)

TV Licence : Jan 2011-Feb 2013(myself)

NHS treatment / appointment lettes, UK baby born birth (April 2012)

Could you please enlighten us how case worker deal above cohabitation documents?

Do we need to show 6 documents since my wife joined (Dec 2010) onwards?

Do we have any templates to show cohabitation spreadsheet?
