Dependent Visa questions (urgent)

Hi all,

My partner has just applied for his sponsored visa online and I plan to apply for a dependency visa.
I have a few questions regarding the process.

1. Do I have to initiate a separate online application for my visa? Or do I submit my documents along with his application?

2. We are not married but have been living together for many years. What documentation can you suggest would be helpful for proof or relationship?
- We have proof of address dating back more than 2 years. We have proof of a couple of different addresses but only 1 or 2 letters for each address, is that enough proof for each residence?
- We have a joint bank account (less than a year old)
- We do not have any utility bills
