ILR Application-Assistance Required

I am working on my ILR application and currently i am on tier-2 visa. I came to the UK on tier-1 but due to some issues i have changed it to Tier-2. I have completed my 5 years and ready for ILR application. I had taken all my leaves when i was on Tier-1. Do i still have to submit a letter from employer for my absences. Also help me if you have a format for letter.
