Tier 1 General Spouse Form

Dear All,
I was wondering if any one can help.
I am on Tier 1 general- got my extension for another 2 yrs done last week.
I got married in my last visit to pakistan in decemeber12 and now planning to apply for spouse visa for my dependent.
Can any one please help as to which application form i need to fill in, as the online login does not shows any category on tier 1 general application form,
or is it VAF10 (december 2012) form to print and hand-in in garrys office in lahore, pakistan,

Your help will be truly appreciated.

Purpose of Application > PBS Tier 1 General
Type of Application > Tier 1 (General) Partner
