Life in the UK Test: Insufficient Photo ID?

I was set to sit my Life in the UK test this morning, but was declined by the invigilator. The reason was my identity document/photo ID wasn't on the approved list. I registered with my UK residence permit (limited leave to remain visa / "immigration status document") - the sticker in my passport. However, there's apparently an A4 sized supplementary document that's similar in appearance to the passport sticker that would've been acceptable, but this is the first time I'm hearing about it and I was never issued one with my visa. I did note that the passport sticker image was on the list of approved documents, but for whatever reason it wasn't passing. I was told I should've used my passport number, but the passport with my UK residence permit is expired and cancelled (visa pages still in tact). My current passport is with the DVLA.

Could someone clarify what happened here or did the invigilator make a mistake?
