PIO APPLICATION FOR MINNOR

Hi Guys

I have few questions about PIO application

a) 8.Nationality (at the time of birth): My child born in UK before we getting ILR and do NOT hold Indian passport , What I need to mention here?

b) what does 'To be fiiled in Duplicate' mean ?

c) do both parents needs to sign declaration? or only thumb impression of child is enough?
