can someone tell me clearly as I am confused now, If I am Polish, live and work on in the UK, no family, applying for permanent resident.
I have all documents/contracts etc

do I have to send
- 60 bank statements - to cover 5 years 12 bank statements a year (or I dont have to send it or a few statements only?)
- do I have to send EHIC card? health card for EU?
- do I send a newest passport issued 2012 or two passports (issued before 2012 and after?
