Arrange Document with covering letter?

Can anyone suggest how to arrange all the document with covering letter. I mean what should be the content of covering letter and then do we have to index all document. And do we have to send a return envolpe along with it. I am applying from UK. All inputs will be appreciated. May be I am being a chicken applying by myself
