Tier-1(G)/Dependant form clarifications ?

K6 - In which country did you obtain your visa ? What date was your visa clearance issued ?
K8/9 - What date was your visa entry clearance valid from ?

Background : Myself and my dependant came here on a Tier-2 visa and switched to Tier-1 on Apr '2011.
So, on what is the answer they expect for that question, Is that related to the current Tier-1(G) or the earlier Tier-2 visa.

This reply says that it related to the current visa, which I highly doubt, Can someone help me on this query ?

the Question B13 on previous address history, do i provide overseas address also or only UK address?
Whn I submitted my Tier-1 I gave my 5 years of address, including overseas. Now do I submit only UK or overseas also ?
